SalesforceGenericException ??

For a week now, our web-to-case forms have been generating a "SalesforceGenericException." Soon after submitting a form to the web-to-case servlet, I can see from the debug logs that the Cases are being created, and the CaseNumber field is being incremented - but the case never seems to be inserted, or at least visible in the UI. Then sometime later (minutes to hours) we get an email "alert" with the text below -- it's missing any useful information...

Any ideas? This is really causing us a headache...
