1. Click on:
Setup | App Setup | Customize | Home | Home Page Components.
2. Create a new custom component and follow the directions to save an image. Make sure to check the box to make the image "externally available". If it is not externally available then you will not be able to select the image.
3. Once the image is saved, click on the "Home Page Layouts" link located under "Home Page Components".
4. Create a new layout and click "Save".
5. Ensure that 'company logo' is selected, then click "Next".
6. In step 2, move 'company logo' to the top of the left column (Narrow).
7. Click "Save & Assign" to assign this page layout to the profiles you would like to have the logo displayed for.
If you are using a Firefox browser, you may not be able to successfully upload images to Salesforce. You will need to use Internet Explorer when uploading your images.

If you mean to replace the Salesforce default logo with your own, this is how I did it:
Setup
App Setup
Create
Apps
Edit or New (insert the image (that you have already loaded to the Document Library and made externally available).

Dear All,
you can make a new application from "setup" link in the left upper corner of page
App setup >> Create >> Apps
then choose "New"
make you application, then add you logo, and add selected tabs such as Home, Leads, ... etc

Is there some kind of issue using Vista? I've tried selecting both My Documents and Public documents and no files are found. The image is in .GIF format and less than 20K in size. Why can't I find it to upload?
