Converting Leads in the NPSP

I've installed the Contacts & Orgs 1.59 in my sandbox so I can get the hang of bulk uploading of contacts, etc. I have a bunch of leads in there with emails and phone numbers, but when I try to convert an individual (putting "self" in the organization slot), I lose all the phone numbers and emails.
I thought I followed all the steps in the after installation instructions. Can somebody tell me where I've gone wrong?
I'm using the one-to-one model with individuals.

Not sure what your problem is. The Convert Lead button needs to be overridden with the VisualForce page.

Ah, yes. Overridden with the VisualForce page. But no indication of WHERE that mytical page might be located. Here's what the documentation says:
I'f you're using the One-to-One model, you'll need to override the Lead Convert button with the OverrideLeadConvert page.
- Leads with accounts convert normally.
- Leads that you want to map to an individual account in the One
to One model will map correctly IF you put in to the Company field one
of the following:
- "Self"
- Last_Name & " " & First_Name
With any one of those values in the Company Field, the contact will map correctly on conversion. So, if you want to use web2lead, you can have your form auto-enter "Self" in to that field if it is left empty.
From the structure, it seems like the 1 and 2 are instructions about how to override the ConvertLead Button. But...There's nothing there about where one might find this page nor how to override the button with it. I'd LOVE some help on this one. I've obviously done it wrong, since it doesn't work in my sandbox. I lose both the email address AND the phone number.

Thanks, Steve. I started following those instructions and realized they must have either been THERE before or somewhere, because I'd already done all that. I'm thinking my problem may be in the phone and email section that follows the lead conversion section of the post-installation instructions. It's got something to do with taking the "Phone" field out of the Lead and getting it into one of the several phone fields on the contact record and then doing the same with "Email."
I'm working on it. Thanks for your help.
